Trinity Center’s Housing Focused Case Management is designed to prepare individuals for a successful housing search and lay the groundwork for positive outcomes when housing opportunities present themselves. The program provides support before, during, and after housing is secured with the goal of maintaining stable housing. The case managers work closely with each individual to assess their needs, develop a customized housing plan, and connect them to resources and support services to achieve their goals. The program is focused on empowering individuals to take control of their lives and break the cycle of generational poverty and homelessness.
